A parent, Nurlina, said on Wednesday that she was shocked when her son showed her the six-page form.
She said page five of the form, on reproductive health, contained graphics depicting breasts, vaginas and penises, and under each of the images, there were four options ' from small to large ' that a student needed to circle.
She said that it was unethical for teenage students to be asked such questions, especially considering that Aceh applied sharia law.
Meanwhile, a school committee member, Iskandar Muda, said the questionnaire was sent by the local health agency through the health community center.
'We are simply distributing the questionnaire. We do not have the authority to stop its circulation,' he said.
